This DD was released in both etched-label and white-label and also with the more traditional sleeve.

 

There was an associated Trade Card as well, but I am uncertain as to the timing of any.

Thank you for the information Rod.  I knew the recording was issued with the white paper label but did not know about the sleeve variation or the trade card.
